Key elements of Wheelchair Assessment include:Background information: This includes basic demographics, medical diagnoses and associated conditions past or planned surgeries, current therapies, vocation or occupation, leisure activities, family and caregiver involvement, home and work environments. Client and caregiver goals: These include both short and long term goals. Current level of function: Range of motion, muscle tone, reflexes, motor control, vision, hearing, cognitive status, communication, skin integrity, pain problems, and ability with activities of daily living (ADLs) are assessed. Seating: This includes a complete evaluation of existing seating and the attributes of the system that are working well and those that are problematic. A full evaluation out of the wheelchair and seating simulation provide optimal information. Mobility: This includes a review of the client's current mobility and a determination of functional levels. Different types of equipment are recommended based on ambulation skills, balance, upper and lower extremity function, cognition and endurance. Wheelchair Prescriptions

The positioning and mobility assessment include a medical, psychosocial, and functional history along with a complete physical examination. 
At Neurocore, we have physiotherapists on staff who are registered authorizers with the Assistive Devices Program (ADP) for Ambulation Aids, Wheelchairs and Positioning Devices/Seating by the Ministry of Health & Long-Term Care. Our authorizers have the experience to appropriately and quickly complete the application forms for those applying to the Assistive Devices Program (ADP).
Our staff believes that those who require assisted mobility should know how to use it and use it safely to enhance one's quality of life and overall well-being. We have created a unique wheelchair training program, which teaches people the vital skills they need to be in control of their wheelchair.
